Student Flu Status Form

Login to the Student Flu Status form.

To help us keep track of students who are experiencing flu symptoms:

1. Go to the Student Flu Status form.
2. Fill out the form to indicate your status, location, and contact phone number.
   (By providing a contact number, appropriate University personnel will be able to follow up with you to see how you are doing.)
3. Visit the UHS website for helpful information and resources regarding the flu.
4. Once you have recuperated, please go back to the Student Flu Status form to update your status.
